Step 1: Prepare an Undertaking/Affidavit in Original (Duly Notarized)

Draft a formal statement, known as an affidavit, confirming your decision to change your name and religion. Ensure that this affidavit is duly notarized by a legal authority or a notary public. The affidavit should be prepared on a stamp paper with a denomination of either Rs. 10 or Rs. 20.

Complete the official form for the public notice, typing your old name and signing it only with your old name. Also, include details of two witnesses who can confirm your identity, and submit 2 copies of this certificate along with the application.

Step 3: Publish a Public Notice in a Local National Newspaper

Place an advertisement in a daily local national newspaper, either in English or Hindi. The notice should include your old name, gender, father's name, full residential address, new name, and gender. Ensure it's from your perspective, not from a lawyer, and appears in the Public Notice section, not in the Change of Name section. This advertisement in original must be submitted with other relevant documents.

Step 4: Prepare a CD

Create a CD containing the prescribed format for the public notice for the change of name in MS Word, excluding the witness section.

Step 5: Provide a CD Certificate Signed by the Applicant

Include a certificate, signed using your old name, stating that the contents of the hard copies and the digital copy are identical.

Step 6: Submit Two Recent Self-Attested Photographs

Attach two recent self attested photographs of yourself.

Step 7: Include a Self-Attested Copy of Photo ID Proof

Submit a self attested copy of your photo ID proof, such as Aadhar Card, Voter ID Card, or Passport.

Step 8: Provide ID Proof for Witnesses

Include self attested photocopies of the ID proof for the witnesses, such as Aadhar card, Voter ID card, or Passport.

Step 9: Attach Conversion Certificate regarding Religion Change

Provide a Conversion Certificate regarding the conversion of religion from a Mandir, Masjid, Monastery, Gurudwara, or Church, issued by the concerned state as per your address. Ensure the certificate contains essential details like registration number, signature and stamp, conversion date, old name, new name with S/o, D/o, W/o along with address. Note that certificates issued from societies, Wakf boards, trusts, and educational institutions will not be accepted.

Step 10: Fee Payment

Check with the Department of Publication for the applicable fee and payment instructions. Pay the required fee on https://bharatkosh.gov.in/.

Step 11: Write a Request Letter

Draft a letter addressed to the Controller of Publication, Department of Publication, with your contact details (telephone number and email). Submit this letter along with all above documents in hardcopy with CD.
