  1. That I am the citizen of India.
  2. That my name has been recorded as {Applicant Old Name} after marraige in all other relevant documents
  3. That now, I have changed my name permanently to {Applicant New Name} in place of my previous name {Applicant Old Name} in view of Divorce from my husband
  4. That in future i will be known by new name i.e. {Applicant New Name} for all the purposes
  5. That I have got divorce from my husband vide Court Decree No. {Court Decree No} and date of disposal on {Divorce case Disposed Date}
  6. That I further declare that both the names mentioned herein above belongs to one and the same person i.e. myself.

Advertise in a daily local national newspaper, ensuring it adheres to the prescribed format. Include old name, gender, father's name, full residential address, new name, and gender. Advertisement must be from an individual, not from a lawyer.This advertisement in original must be submitted with other relevant documents.

Attach two recent self-attested photographs of yourself.

Step 8: Provide ID Proof for Witnesses

Step 11: Write a Request Letter

Draft a letter addressed to the Controller of Publication, Department of Publication, with your contact details (telephone number and email).

Sir/ Ma'am,
