  1. Name Change Affidavit

You might need additional papers, like a marriage certificate or a gender change certificate, depending on your situation.
  2. Prescribed Proforma for Change of Name
    Next, you'll fill out a two copies of prescribed proforma with your old name. Get two witnesses details with signature and sign it using your old name, confirming your identity. This step helps formalize your request.
  3. Newspaper Publication
    Tell the world about your name change by publishing a notice in two local newspapers. One in your state's official language and another in English. Keep copies of these notices, they're proof that you've made the change public.
  4. Compact Disc
    Write a CD that includes MS Word format of the prescribed performa but without the witness section. 
    Note: Instead of signature, your old name should be written at the end.
  5. CD Certificate
    A CD certificate (signed using your old name) stating that the hard copies and soft copy are identical.
  6. Photographs and IDs
    Attach two recent photos of yourself along with a copy of an ID (like Aadhar Card, Voter ID, or Passport). This helps confirm your identity.
  7. Witness IDs
    Include copies of the 2 witnesses' IDs (Aadhar Card, Voter ID, or Passport). This ensures they're legit witnesses.
  8. Request Letter
